Rehabilitation (current) The reader should be sure to have read Part 1 of this series as it gives useful background needed for reading this Part. Fibrosis and oxygenation As discussed in Part 1, the damage that is done can occur not only during the surgery but in the period of time after the surgery due to a build up of fibrosis in the penis blocking nitric oxide (NO) and so erections. Such fibrosis can represent irreversable damage. By achieving erections as soon as possible after catheter removal (within 6-8 weeks), oxygenation will occur and fibrosis may be prevented. In [Endotext] this is referred to as "avoidance of penile hypoxia through regular tissue oxygenation via erections". Wont Although it does not appear to have been specifically studied, the various treatments are not mutually exclusive so simultaneous modalities may be considered, e.g. exercise + drugs + VED This article by Sidney Radomski is only one page and summarizes what works and what does not work: [link]. Readers are encouraged to read it before reading on. Other sources of information include the Phoenix5 site [link] and a 25 page article outlining various treatments by Drs. Stephen Auerbach and Aubrey Pilgrim [here]. Exercise Exercise consists of attempting to achieve erections possibly with the aid of any of the following treatments. "The use of at least one aid was an independent predictor of more favorable sexual HRQOL" (higher quality of life). [PMID: 16844457]. Frank Sommer, a German urologist, has a set of exercises which he claims are just as effective as Viagra (though they do not appear to have been specifically tested post prostatectomy): See [men-and-health] and [urotoday] for information on the study. The first link in the last sentence also provides a description of Sommers' exercises. Note that Sommers' exercises are different than the incontinence exercises discussed in [this post]. There may be a benefit of exercise prior to surgery as well although we have not found a source where it was clearly established. PDE-5 Inhibitors Viagra (sildenafil), Levitra (vardenafil) and Cialis (taladafil) are drugs which inhibit PDE-5 which breaks down the key promoter of erections, cGMP. This indirectly enhances and prolongs erections provided one can achieve at least partial erections already. Daily Administration . It can work in conjunction with appropriate exercise to achieve more effective erections which may enhance the rehabilitation process. For rehabilitation purposes these drugs are taken at a low dosage every other day or other frequency although some good results have been reported by taking them every day. This [PMID: 16766116] discusses a successful trial of daily administration of Cialis and the review article in the following link discusses various good results in using PDE-5 inhibitors on a daily basis rather than on demand for ED, cariovascular problems and other problems: [PMID:1746047]. Table 1 of this last paper summarized the Benefits and Limitations of daily administration for ED as follows. Benefits: salvage of on-demand responders, improved treatment response in difficult-to-treat groups, disease modification, may approximate more natural sexual function, randomized placebo-controlled trials demonstrate efficacy and safety for ED of various etiologies. Limitations: extended term follow-up limited -- safety and efficacy profiles require further elucidation, mechanisms of treatment effects incompletely understood, disease modification -- attractive concept but evidence-based data limited, patient cost, no evidence of tachyphylaxis to date but longer follow-up needed. (Tachyphylaxis refers to decreased response after initial administration requiring drug-free periods.) A 2008 paper in the Journal of Urology found that patients who had better pretreatment sexual function responded better to PDE-5 inhibitors. [PMID: 18206926]. Mulhall's Approach . In a Medscape article [Penile Rehabilitation Following Radical Prostatectomy] (see section entitled Structure of Rehabilitation) and 2006 Urology Times interview John Mulhall (papers) indicates that there is animal evidence for daily administration (every other day for Cialis) preoperatively as well as post operatively and so uses the following protocol for his patients (quote is from Medscape link): I encourage presurgical patients to use low-dose PDE-5 inhibitors on a nightly basis for 2 weeks before their operation. This strategy is based on the animal data supporting pretreatment. These patients are then told that with the catheter in place they should continue to use low-dose PDE-5 inhibitors on a regular basis (sildenafil and vardenafil nightly, tadalafil 3 times a week). When they are given the go-ahead to resume attempts at obtaining erections, they are switched to a low-dose PDE-5 inhibitor 6 nights a week and a maximum dose 1 night a week. The maximum-dose pill needs to be used in an appropriate fashion with sexual stimulation. The patients are encouraged to return to the office 6 weeks after surgery, which will allow them approximately 4 weeks to try maximum-dose medication. After 6 weeks the protocol is as follows. In the exceptional case that this is successful in restoring sexual function within 6 weeks, they can continue with PDE-5 inhibitors. In the more likely case that they are not successful within 6 weeks then therapy is switched to intracavenosal penile injections twice a week (with appropriate training) and low dose PDE-5 inhibitors on non-injection nights. Because of its longer half life Cialis is not appropriate for this phase of the therapy. After a year they try maximum PDE-5 dosage once a month and if successful on that can stop injections. He normally expects some improvement within 10-14 months and optimal functionality in 18 to 24 months and notes that failure to respond to PDE-5 inhibitors in the first year does not necessarily preclude an excellent response after two years. The link cited above also gives a variation of this protocol in the case that it is started post-operatively. Comparison . A key difference among the drugs is that the half lives of Viagra, Levitra and Cialis are such that they last 2-8 hours, 4-8 hours and 24-36 hours respectively. The longer lasting Cialis would appear to be superior from the viewpoint of rehabilitation and also can be used at the lowest concentration which may make side effects less likely. However, there have been claims that Viagra is more effective at achieving intracellular drug concentration and cGMP accumulation as well as one study that claims that Levitra results in higher concentration [PMID: 15213306]. Others have indicated that there is individual variation and if one of the three drugs does not work to try the others. Detailed comparisons of the three are in the table at the bottom of page 2 in this link Greenspan, 2004 and Table 1 of Mehrotra, 2007. A February 2008 study concluded that patients who were given a chance to try all three and choose for themselves had better compliance with taking this medication [PMID: 18086159]. A November 2007 review of PDE-5 inhibitors by Carson can be found here: [PMID: 17983891] [Full Text] also mentions that coffee is a PDE-5 inhibitor; however, coffee may have an adverse affect on incontinence and its pro-inflammatory properties might also promote recurrence. (As an aside we mention that PDE-5 inhibitors may have benefits against hypertension and other vascular diseases and are also believed to enhance one's ability to work in a low oxygen atmosphere such as would be experienced on mountain tops or by pilots.) Injections Although these drugs are desirable from the viewpoint of not being invasive, because they do not generate erections but simply block inhibitors they may not be as effective in the earlier stages of recovery as injections into the penis which directly generate the nitric oxide (NO) needed to begin the chemical process that leads to erections. According to [PMID: 16158022] 55% of prostate cancer patients treated with prostaglandin E1 injections felt that their sex life improved. More information on injections can be found at the Phoenix5 site [link] Vacuum Erection Devices Vacuum erection devices (VED) are mechanical devices that use suction to draw blood into the penis resulting in an erection. They are effective although one gets a cold erection and can be a nuisance. A ring is used to prevent the blood from escaping; however, simply for the purpose of achieving erection it would be good enough to pump up without the ring on a regular basis. There is some debate on whether or not VEDs provide oxygenation or not. An instructional video showing how these vacuum devices work can be found [here] and comments by users are found [here]. VEDs can also be used for rehabilition. In [PMID: 17822466] investigators perform a randomized control trial which concludes that daily use of a VED starting one month after prostatectomy signficantly "improves early sexual function and helps to preserve penile length". The following earlier work also supported the fact that early VED use can preserve penile length [PMID: 17657210]. If one can achieve erections it may be possible to prolong them simply by using a silicon ring. This is inexpensive and easier to use than a VED plus a ring. A testimonial on its effectiveness is found here: [link]. The ring itself is unlikely to provide oxygenation and might even interfere with oxygenation if left on too long so its only use would be to allow sexual activity. Other treatments . The New York Times Reports “Good News” About American Health Care

That’ll Be The Day “All I know is just what I read in the papers.” Will Rodgers,1879-1935 I await the day when The New York Times runs a series of “good news” articles about the state of American health care. The series might have these titles, • Americans Trust Their Doctors • Americans Have Greater and Quicker Access to High Tech Diagnostic and Curative Care Than Any Other Nation • Foreign Physicians Flock to America for Training Unavailable in Their Country • Record Numbers of Canadians Cross Border for Life-Saving Care • America Achieves Unprecedented Longevity Gains in Last Decade • Americans Receive 80 Percent of Noble Prizes in Medicine • Research at American Pharmaceutical Companies Produces 90 Percent of the World’s New Drugs • America’s Innovative Health System’s Variety and Choice the Wonder of The World That’ll be the day. The Times in 2005 and 2006 had a series of a dozen articles entitled “Being A Patient.” These focused largely on the perils of being a patient in America. Now The Times is embarked on a series on medicine and money, focusing on profit-mongering drug and medical device companies in league with greedy specialists to bilk the public. It all comes down to altitude and attitude. From their lofty perch, Th e New York Time’s editorial staff has yet to tumble to the reality America is basically a conservative nation, distrusts centralized government, wants choices of care and providers, demands access to the wonders of high tech medicine, and believes a market-based system, with all its faults, such as profits for entrepreneurial and innovative health care companies and , are worth the price and value received. It is almost as though The Times denies the existence of entrepreneurial capitalism in American health care. Our health system blends innovative large and small firms striving for economic growth. Such a system entails risk – workers who lose jobs and health insurance, widening of gaps between winners and losers, competition with some jobs going to skilled workers abroad who have increasing skills, occasional bankruptcies among those unable to pay health care bills. American capitalism is imperfect. It requires oversight to reduce risks without losing entrepreneurial vigor. Unremitting accusations of bad faith and constant “bad news” stories don’t strengthen health care. Read the The New York Times, and you’ll come away believing pervasive avaricious greed corrupts American health care and will break our already “broken” system. From May 9 through May 11, The Times ran 10 articles on how drug companies deceived the public and entered into unholy alliances with doctors to sell more drugs to produce more revenue for doctors, how doctors willingly entered into these alliances solely for material gain, and how lobbyist-tainted and incompetent FDA failed to monitor new drugs and harmed patient safety. The May 9 front page, right top column, the prime spot for highlighting news, featured these headlines, Doctors Reaping Millions for Use of Anemia Drugs. Payments from Industry. Concerns over Safety – Critics See Incentives for Higher Doses. The opening Section read: “T wo of the world’s largest drug companies are paying hundreds of millions of dollars to doctors every year in return for giving their patients anemia medicines, which regulators now say may be unsafe at commonly used doses. The payments are legal, but very few people outside of the doctors who receive them are aware of their size. Critics, including prominent cancer and kidney doctors, say the payments give physicians an incentive to prescribe the medicines at levels that might increase patients’ risks of heart attacks or strokes. Industry analysts estimate that such payments — to cancer doctors and the other big users of the drugs, kidney dialysis centers — total hundreds of millions of dollars a year and are an important source of profit for doctors and the centers. The payments have risen over the last several years, as the makers of the drugs, Amgen and Johnson & Johnson, compete for market share and try to expand the overall business.” The Times appears bent on publishing on its front pages “All the Bad News that’s Fit to Print about U.S. Health Care.” The May 9 article is part of a series of medicine and money, all decrying collusive relationships between big business and bad doctors. The Times series focus on the pharmaceutical industry and medical device industries , and how these industries reward specialists who overuse products for financial gain. To The Times, the American health system has become a morality play, • the good guys (The Times and other assorted elites and policy pundits) vs. the bad guys (profiteering health companies and doctors); • the greedy (well-healed executives and “rich” doctors) vs. the needy (poor patients in the throes of cancer or kidney dialysis); • the high brows (academics and journalists who know what’s right for the common good) vs. the low brow commercial types (who do almost everything wrong as long as it suits their own financial self-interest). I don’t wish to pick a fight with a media outlet who buys ink by the barrel. I know “bad news” sells better than “good news.” I know The Times considers itself the Watchdog and Whistle-Blower against mean-spirited, profiteering conservatives. I don’t question our capitalistic system needs oversight to reduce abuses. I’m simply seeking more balance in The Times reporting. For an example of this imbalance, in its May 9 piece, The Times dismisses America doctors’ overuse of anemia-correcting drugs for cancer and dialysis as a deliberate effort to make money. To make its case, The Times notes American doctors, • prescribe more drugs than European counterparts ( Did it ever occur to T he Times maybe, just maybe, European doctors “under-prescribe” and maybe their patients have less positive results? ) • conssciously endanger patients for profit when they know anemia drugs are unsafe (Has it occurred to The Times American physicians prescribing these drugs believe higher hemoglobin levels are “good” for improving health and alleviated distressing symptoms attributable to anemia.) • Continued to prescribe drugs even after studies indicated hemoglobin levels above 12 might endanger patients ( Did it ever occur to The Times the studies indicating “possible” risk studies were far from conclusive and only appeared in March?) Nor does The Times point out doctors themselves often criticize thenselves. For instance, on a May 11 blog, “The Doctors Weighs in on Cancer,” Dr. Dov Michaeli, an academic physician and biochemist who does cancer research takes the American Society of Clinical Oncologists (ASCO) to task for responding to the Times defensively (see epilogue to this blog for a reprint of ASCO letter to The Times). Of the ASCO letter to the times (reprinted in epilogue), Dr. Michaeli acidly comments “ASCO makes that same argument that professional people make when colleagues are caught with their hands in the cookie jar: most of us are conscientious, hardworking people. Granted, but it turns a blind eye to the corrosive influence of pharmaceutical companies on the use of drugs. This is denial of how our health system ‘works’ on a daily basis.” Michaeli concludes: “As the wheels are coming off our broken health system, more revelations of waste, abuse, greed and outright criminality are bound to surface. What are we going to do about it?” Good question. I suggest we start with a more balanced view of the system. • First, I reject the notion the system is “broken” – and constant reference by academic critics of greed by practitioners as a cause for this brokenness ( Michaeli, an academic researcher, shows some of this bias when he says, “ ASCO is led by academic clinicians and researchers, whose motivation and dedication is admirable. But many of the rank and file, community practitioners, are not beyond temptation.” I doubt medical academicians, who compete for pharmaceutical company grants and who run clinical trials, are beyond temptation. I’m unaware academic physicians wear halos and only practicing doctors are vulnerable to “temptation.” • Second, I believe critics ought to acknowledge health care is an innovate force in our economy, will soon represent 20 percent of the nation’s GNP, and is the nation’s largest employer. Professional managers, whose job is to maximize resources and revenues, run most health care enterprises - hospitals, medical practices, drug and device manufacturers. If overzealous pursuit of revenues and resources leads to excess, managers should be condemned, even fined and jailed, but it shouldn’t be assumed or taken for granted pharmaceutical and medical device companies and doctors are always seeking mutually beneficial arrangements are ipso facto evil doers. What the media in general, and The New York Times in particular, needs is a more balanced view. An occasional dollop of good news, such as more than 50 percent of cancer victims are now surviving, more than 10 million cancer victims are living with their disease, and genetically engineered cancer drugs are contributing significantly to cancer cures, would help achieve that balance. I’m pleased to report the May 12 issue of The Times contains a “good news” piece on Becton, Dickinson & Company. It’s buried on the third page of the business section. It’s titled “Medical Gear That Rarely Makes News.” It consists of an interview with Edward J. Ludwig, CEO of Becton and Dickenson, with revenues of $5.7 billion last year, on sales of syringes, diagnostic kits, lab equipment, and related gear. The unifying theme behind the company’s success is its emphasis on safety in its products to protect doctors, nurses, and patients with shields, sliding clasps, and needle retracting into the device. Its ambition is to make a significant dent in the 2 million infections each year from antibiotic resistant staphococci killing 90,000 Americans each year and costing $6 billion yearly to treat. Toward that end, B &D has acquired a diagnostic system allowing them to quickly identify the offending bacteria. Use of this system to screen every patient. entering Evanston Northwestern Hospital reduced infections by 60 percent. Ludwig contend s private innovation will help the “broken” health system to heal itself by attacking safety problems, and improving care. What the media needs is a new more flexible mindset allowing them to become more innovative in reporting the “good news” of our resourceful and responsive health system. Epilogue : In the interest of being “fair and balanced” (a term the mainstream media now considers anathema since Fox News adopted it as their slogan), I reprint six letters from the May 13, Sunday, New York Times. The Times deserves credit for publishing letters representing both points of view. Best Drug, or Best Money Maker? (6 Letters) 1) To the Editor: So two drug companies are paying hundreds of millions of dollars to doctors who prescribe anemia medicines that lack effectiveness and put a patient’s health at risk. This is not a surprise because it reflects our broken health system, a system driven by greed. Although drug companies say their intentions are not to promote the use of more medicine for profit, there will always be the risk that some doctors will prescribe higher doses to gain that extra dollar. As patients, we should work to eliminate the incentives to doctors and to raise patient awareness about them. We deserve the right to know the benefits of a medicine, both for us and for the doctors. Luis Rodriguez Daly City, Calif., May 9, 2007 2) To the Editor: Medical care should be guided only by what is best for patients. But throughout the medical system, rebates and volume discounts are common and can create the perception of improper incentives. Our organization has long advocated evidence-based guidelines, including those we produced in 2002 with the American Society of Hematology on erythropoietin use for chemotherapy-related anemia. With the appropriate use of erythropoietin, many thousands of patients have avoided potentially dangerous blood transfusions. Oncologists care deeply about their patients, and the overwhelming majority treat them based on the best available evidence. In the case of erythropoietin, recent studies prompted the Food and Drug Administration to issue a “black box” warning in March about the potential dangers of using erythropoietin to boost hemoglobin to levels higher than guidelines recommend. Early evidence suggests that doctors factored this new data into their prescribing decisions and have reduced erythropoietin use. As a whole, the medical community needs to better determine the impact financial incentives may have on prescribing patterns and patient care, to ensure that patient needs continue to be at the forefront of medical decisions. They can and will prescribe for personal profit, and will readily reshape and expand diseases to suit the available reimbursement. Without disclosure, patients are typically the last to know there might be a problem. The investigation of anemia drugs no doubt could expose the self-serving logic, unethical inducements and poor administrative surveillance that permit exploitation of the public’s soft financial underbelly. Unfortunately, there are plenty of other specialties of medicine where such professional betrayals occur. And adequate regulation is not likely to occur in the financial free-for-all of private medicine. James H. In the appropriate setting, these growth factors can improve blood counts and quality of life and spare patients time-consuming, expensive, short-lasting and risky transfusions. In our practice the increasing use of these medicines is driven by the fact that they work so well. As with any new therapy, these medicines need to be used within established and developing guidelines to avoid serious side effects. Since there are two competing and equally effective drugs, the drug makers are offering incentives for preferential use — the natural outcome of a free-market economy. Deciding how regulators might control drug makers is an important undertaking, but it should not detract from the tremendous benefits of these drugs when used in the right situation. It is the most advanced and expensive. The first two qualifications are debatable, but the third is difficult to refute. The great expense is complicated by the high cost of drugs and procedures of dubious benefit. The likelihood of being prescribed drugs of dubious benefit is obviously increased by kickbacks to doctors. The kickbacks may be legal, but should they really be allowed? The cost of medicine is increased by this practice, and the quality is sure to suffer. I found it equally disturbing that the continuation of the article was in Business Day. In the past two decades, I have observed that news of important medical advances increasingly appears in, or is continued in, the business section. This practice advances the thinking that health care is primarily a business in which providers reap riches, rather than a humane social endeavor in which providers earn their living. Ira D. I remember sitting in the waiting room with about 20 other candidates, and someone was frantically flipping through a textbook on X-rays, while another person described their experiences teaching blind leprous children in India how to read. I felt a bit unprepared. I referred to that memory as I sat as the student member on one of the interview panels a few weeks ago. The medical school encourages student participation in the admissions interviews, and I believe this to be an essential part of ensuring a fair assessment of each candidate as an individual and not just a set of grades. It is also part of the BMA Medical Students Committee guidelines for Medicine in the 21st Century, issued in January 2003, which state that

Week 3 of Winter Quarter

Peugeot 404 It was nice to have MLK off but it really meant condensing all the events and business and studying that we had to do in a limited amount of time. Some of my classmates are doing introductory pharmacy practice experiences, like following around our past Clinical Pharmacy professor, Kathy Dennehi, while taking medical histories/ screening for immunization eligibility. It is apparently a pilot program that was jumpstarted so that UCSF Pharmacy School could claim that at least 5% of its curriculum is dedicated to IPPE's. They can use this to leverage with the Board of Pharmacy to get reaccredited. I think that it is a great idea to have something in the pharm.D. program to introduce students to applying these skills that they teach you in school about assessment and counseling. From my experience in retail, you will eventually learn some patient counseling, but IPPE's can give you a much easier time during rotations and not have to climb such a rough transition from textbooks and lecture notes to real life therapeutics and patient interaction. We took our first midterm today and it was not too bad. I actually had a harder version of biochemistry in undergrad and this class was more emphasized on getting you to understand the clinical correlate of biochemistry instead of learning every annoying enzyme for those who want to become lab scientists. Our professor is really great; her name is Tracy Fulton. One class that I really enjoyed was the clinical correlate taught by our Dean, Mary Anne Koda Kimble in conjunction with Tracy Fulton. She had a mock interview with a diabetes patient while we learned about insulin and glucagon control of metabolism. One interesting point was on sulfonylureas, which increase insulin secretion from pancreatic beta cells by blocking membrane bound potassium leak channels. She even went on to explain how diabetes patients often get foot ulcers, from poor circulation and poorly functioning clotting factors. APhA had its first legislative committee meeting which assigned us first year members our own projects towards promoting the profession. My second visit as a volunteer to Laguna Honda Hospital was much longer than the first and incredibly worthwhile. I walked in to the main hall feeling like a thousand eyes upon me, knowing that I was out of place in a room of elderly patients with dimentia. I was silent as he played Bingo for about half an hour because I was afraid if he could speak coherently, percieve and be capable of interacting with me, or even have a shred of short term memory. To my surprise, he asked me very clearly what my name was and proceeded to enter a perfectly normal conversation. He was able to respond and converse with me more capablely than many of my contemporaries. So apparently, Robert was first admitted into LHH when he overdosed on alcohol. Ever since he's been residing there. In the past, he recieved his first dirtbike at the age of 11 and rode on racetracks and jumps while attending a baptist church. Throughout his life, he has traveled the entire country. Born in Chicago, he was raised on a farm in Arkansas by Swedish and Irish parents. He became a professional motorcycle racer and came in 3rd nationally in 1968. Behind the public image, many bikers spent their play money to shoot up on heroine and coke, and he was not exception. I'm not sure about what he did in New Orleans but eventually he made his way to San Francisco. He tells me that his Swedish father, a retired stockbroker, is still alive somewhere in America. So hearing him recall his life as a hyperkinetic zigzagging path, transcending state lines, dotted with 151 proof rum, irish carbombs, and car girls. It was a perfect description of an existence that epitomized the American ideal. Living with the freedom to be susceptible to sin, ut have enough of a safety net to recover, roam but always have ties with loved ones in the past, and connect with the land but not to be tied down to it. And I think it is such an important concept to travel in order to understand America but not objectify it. His middle class values reminded me of my own father. I haven't heard anyone say the word America so many times with such gravity and with such respect that only the wise can grace upon it. It was not a conversation of problem solving or thesis writing, but it was the most intellectually stimulating conversation that I have had in a while, much moreso than with classmates whom have master's and all sorts of degrees and scholarships under their belts. His gaze started to drift and his attention dissipated as he felt a craving for a cigarrette. He bummed one off of another resident and we stepped outside so he could smoke it. He began to repeat his entire story to me as if I had never heard it before. Then I suddenly got the sense that he was not as aware of me as I was of him; the feeling you get when the person you are trying to interact with does not remember you hurts.
